Does Pennsylvania Workers’ Compensation Cover Hip Replacement Surgery?
June 19, 2026 | Workers’ Compensation | Christopher J. Marzzacco
If you’ve suffered a hip injury on the job in Pennsylvania, your doctor may eventually recommend hip replacement surgery as part of your treatment plan. This can raise a lot of questions, including when it comes to whether workers’ compensation will actually pay for the procedure.
The short answer is yes. Pennsylvania workers’ compensation may cover hip replacement surgery, but only under certain conditions. Insurance carriers frequently push back against these claims, which is why it helps to learn about how the process works and what could go wrong along the way.
When Workers’ Comp Covers Hip Replacement Surgery
Your employer’s insurance carrier is required to pay 100% of the medical expenses related to your work injury under the Pennsylvania Workers’ Compensation Act. There is no cap on medical benefits.
Coverage extends to a wide range of treatment, including:
- Emergency room visits and hospital stays
- Surgical procedures, including hip replacement
- Prescription medications
- Physical therapy and ongoing rehabilitation
As long as the hip injury is connected to a workplace accident and the surgery is considered reasonable and necessary by your treating physician, workers’ compensation should cover the full cost.
Why These Claims Are Often Disputed
Although the law says your medical bills should be covered, insurance carriers fight hip replacement claims more than almost any other type.
There are a few reasons why:
- Hip replacement surgery is costly, and the recovery period often means months away from work
- Many workers are left with lasting physical limits after the procedure, which means the insurer could be on the hook for benefits long into the future
- Workers over the age of 50 often have some wear and tear in their joints without even knowing it; when imaging picks up on that, the insurer may try to blame the condition on aging rather than the workplace accident
Note, however, that Pennsylvania law also recognizes what is known as the aggravation doctrine. In simple terms, this means that if your work injury made a pre-existing condition worse to the point where hip replacement became necessary, the surgery should still be covered.
What Is the Utilization Review Process?
Another way insurers challenge hip replacement claims is through a process called utilization review. This is a formal procedure where the insurance carrier questions whether your medical treatment is reasonable and necessary.
When a utilization review is filed, payment for the treatment in question is paused until the review is resolved. An outside medical reviewer evaluates your treatment records and issues a determination. If the review finds the surgery is not reasonable or necessary, the carrier can refuse to pay.
What Happens if Utilization Review Denies My Hip Replacement?
If you receive an unfavorable determination, you have the right to challenge it by filing a Petition for Review within 30 days. This brings the dispute before a workers’ compensation judge for a final decision.
To protect yourself during this process, you should:
- Continue following your doctor’s recommended treatment plan
- Keep detailed records of all medical visits and communications with the insurance company
- Work closely with your treating physician to document why surgery is medically necessary
- Consult with a workers’ compensation attorney who can guide you through the appeals process
Having medical documentation from the start puts you in a much better position if your claim is challenged down the line.
